Medication Reconciliation Reflection

In my project I did notice some safety concerns just in the medications that my client was taking. Since she was on so many antihypertensive medications this increases her risk for developing the adverse effects that these medications may cause. Most of the antihypertensives caused the same adverse effects which would increase her risk even more. The most concerning and most common adverse effect among the medications she was taking was drowsiness which increases her risk of falls and other safety concerns such as driving. I educated my client on my concerns and she was not aware of these safety concerns b ut she stated that she would monitor for any signs of adverse effects and mention these to her provider. After completing the medication reconciliation project for my pharmacology class I learned how vital a nurses role is in making sure that all the medications that their patient is prescribed work well together and benefit the patient. Sometimes there are interactions between drugs and as the nurse it is my responsibility to educate the patient and notify their provider of any findings in order to prevent any negative effects on the patient or their safety.
